3 especially is pretty much a blur.  You could try a university library if
there's a medical school close by, but .........

The Practitioner is published by Miller Freeman Professional Ltd. London,
U.K - its international number is ISSN 0032-6518 and it has been going
since 1868.  

If you are anywhere near Johns Hopkins, you can find it in Stack Level 1,
Welch Library.
